In most cases, no. You can call Sound Advice directly at 818-841-0066 to schedule your hearing evaluation without a physician referral. Some insurance plans, however, may require one before they cover the visit, so it is worth a quick call to your carrier first.
If your primary care physician or an ENT specialist has already referred you, please bring any referral paperwork or relevant medical records — including prior audiograms — so Dr. Lew can build on what you already have.
Please bring your insurance card, a photo ID, a current list of medications, and any previous hearing test results or audiograms you have. If you downloaded the patient forms from this page — or we mailed them to you ahead of time — please bring them filled out. It shaves real time off check-in.
We also encourage you to bring a family member or close friend. Having someone familiar with your everyday listening environment helps Dr. Lew understand which situations matter most — quiet conversations, restaurants, music rehearsals, the car, or video calls.
